Fort Myers Shirts Order Form

Use the Order Form below to order your shirts.  Just click on the “Add To Cart” button and the shopping cart will appear in the right hand column.  In the shopping cart enter the quantities of the item you want.  You order the shirts separately from the prints, just make sure the total number of each is the same.

There is a 12 shirt minimum with at least 1 print on each shirt.  This is for the Gildan 6.1 oz 100% Ultra Cotton Shirts. You can mix the shirt sizes in any combination you want.  The 10 x 12 and 12 x 14 prints can be placed on the front, back, left side and right side.  Include as much information as possible on the “Instructions To Seller” box when completing the Order Form at the PayPal website.  You can also send an email to dennis@FortMyersShirts.com after you complete your order with the printing location information.
